You may fill them out ahead of time and email them to info@visualexpressionsoptometry.com in order to save time at your appointment. If you prefer, you may also print the forms, fill them by hand, and bring them to your appointment. 

 

In addition, if you would like records sent to us from another office, you may fill out the form on the right.
